Flu Shot Appointment Checklist: What to Bring and Ask

Bring your vaccination history if available, a current medication list, relevant allergy and reaction details, and any coverage information the vaccination site requests. Before booking, confirm that the site can vaccinate someone of your age and health status and ask about its appointment requirements.

Confirm the details before traveling

An online vaccine page is not confirmation of an available appointment or the right vaccine for you. Contact the vaccination site and ask:

  • Can you vaccinate someone of my age, and what screening is required?
  • Do I need an appointment, identification, consent forms, or a parent or guardian present?
  • How can I confirm which flu vaccine is appropriate before the visit?
  • What information is needed to check coverage, and could there be an out-of-pocket charge?
  • If I am uninsured, what payment or other vaccination options should I explore?

Ask your insurer whether the specific site is in-network and whether vaccination is billed through the medical or pharmacy benefit. Do not assume that a previous visit’s coverage or price applies now.

See Vaccines and immunizations for local service information, then confirm availability, eligibility and visit arrangements directly.

Gather records and a short health history

Bring a vaccination card or record if you have one, plus the date and location of your most recent flu vaccination if known. For a child, prior flu vaccination history can be especially important when the clinician determines the schedule.

CDC’s pre-vaccination guidance calls for reviewing vaccination history and screening for precautions before every vaccination. It also says missing records should not by themselves postpone vaccination. Tell the vaccinator what you know rather than guessing or assuming you must cancel.

Write down:

  • Prescription medicines, OTC medicines, and supplements you currently use.
  • Allergies and what actually happened during each reaction.
  • Any previous reaction to a vaccine, including when it occurred.
  • A history of Guillain-Barré syndrome or fainting with injections.
  • Current illness, pregnancy, immune-system conditions, or treatments that affect immunity.

Share this information with the vaccinator; the list is not a set of automatic reasons to avoid vaccination. If you are unwell or have a past serious vaccine reaction, contact the site or your clinician before traveling.

Ask about current recommendations, not last year’s product

Ask, “Which current recommendation applies to my age, health history, and previous vaccinations?” The CDC flu vaccination page explains that vaccine suitability depends on age and health, and that timing can differ for some groups. Have the clinician apply current guidance rather than selecting a brand based on an old article or a friend’s experience.

Useful follow-up questions include whether a child needs another visit, whether other planned vaccines affect scheduling, and whether there are special instructions for your medical situation.

Leave with aftercare instructions and documentation

Request the Vaccine Information Statement and ask how long to remain at the site afterward. Before leaving, find out which reactions are expected, whom to call with concerns, and how to obtain a vaccination record for your clinician.

According to CDC flu vaccine safety guidance, soreness, headache, fever, nausea, or muscle aches can occur and are generally mild. Trouble breathing or other signs of a severe allergic reaction are an emergency: call 911. Ask the vaccinator to explain the difference between routine aftercare and symptoms needing urgent help.
